About the Role

The Guildhall & Barrow Surgery is looking to welcome a qualified and experienced clinical pharmacist into our busy practice, to play a full and active role within our multi-disciplinary team. The role is offered on a full-time basis although part time hours may be considered for the right candidate.
 
The main responsibilities of the role include:
  • Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas
  • Consult patients within defined levels of competence and independently prescribe
  • Receive referrals and directed patients from triage services and other clinicians
  • Receive and resolve medicines queries from patients and other staff
  • Provide medication review services for patients in the practice and during domiciliary visits to the local nursing home
  • Manage a caseload of complex patients, as appropriate
  • Manage therapeutic drug monitoring system and recall of patients taking high risk drugs, i.e. anticoagulants, anticonvulsants and DMARDs etc
  • Deliver long term conditions clinics and home visits particularly for patients with complicated medication regimes and prescribe accordingly
  • Provide pharmaceutical consultations to patients with long term conditions as an integral part of the multidisciplinary team
  • Review medications for newly registered patients
  • Improve patient and carer understanding of, confidence in and compliance with their medication
  • Encourage cost-effective prescribing.
  • Implement and embed a robust repeat prescribing system.
  • Provide advice and answer medication related queries from patients and staff
  • Medicines optimisation systems including the repeat prescribing and medication review systems
  • Improve the quality and effectiveness of prescribing through clinical audit and education to improve performance against NICE standards and clinical and prescribing guidance.
  • Develop yourself and the role through participation in clinical supervision, training and service redesign activities
  • Ensure appropriate supervision of safe storage, rotation and disposal of vaccines and drugs. To apply infection-control measures within the practice according to local and national guidelines

About the Candidate

To be considered for this position, you will require the following: 
 
Essential: 
  • GPhC registered pharmacist
  • Hold or be working towards an GPhC independent prescribing qualification
  • Experience in managing pharmacy services in primary care
Desirable: 
  • Membership of the Royal College of Pharmacy
  • Minimum of two years working as a pharmacist demonstrated within a practice portfolio

About Us

The Guildhall & Barrow Surgery is a friendly two site GP practice with a patient list size of approx 13,800. The practice is made up of 6 x GP Partners, 8 x salaried GP's, 1 x Senior Clinical Pharmacist, 2 x Advanced Nurse practitioners, 1 x Nurse prescriber (supported by a team of RGN's and HCA's). We also have a visiting paramedic, pharmacy technician and first contact physio service provided by BSE PCN. We are an accredited GP training practice and our clinical system is SystmOne.
