About the Role

South Rural Primary Care Network are looking to recruit a PCN Clinical Pharmacist to join their established team based at Holbrook and Shotley Practice. Offered on either a full or part-time basis, the successful candidate will be expected to work across both practices.

The post holder will be an integral part of the general practice team, working in each practice and across the PCN, as well as part of a wider pharmacist network and multidisciplinary team. This includes being a conduit of patient medicines information into and out of hospitals. They will work to optimise medication issues to improve patient care and safety and support clinical staff in the management of patients.

Primary responsibilities will include;

Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas
Act as the PCN point of contact for all medicine related matters, establishing positive working relationships
Liaise with the practices and, where practicable, to standardise the medicines management process across the PCN
Consult patients within defined levels of competence and independently prescribe acute and repeat medication
Receive referrals and directed patients from triage services and other clinicians
Receive and resolve medicines queries from patients and other staff
Provide medication review services for patients in the practice and during domiciliary visits to the local nursing home
Manage a caseload of complex patients
Manage therapeutic drug monitoring system and recall of patients taking high risk drugs, i.e. anticoagulants, anticonvulsants and DMARDs etc.
Deliver long term conditions clinics and home visits particularly for patients with complicated medication regimes and prescribe accordingly
Provide pharmaceutical consultations to patients with long term conditions as an integral part of the multidisciplinary team

About the Candidate

You will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications:

GPhC registered Pharmacist
Hold or be working towards an independent prescriber qualification
Hold or be working towards CPPE primary care pharmacy education pathway
Minimum of 2 years working as a pharmacist
Experience in managing pharmacy services in primary care

Our PCN includes the following practices: Bildeston Health Centre, Constable Country Medical Practice (East Bergholt and Capel St Mary), Hadleigh Health Centre, The Holbrook and Shotley Surgeries and Needham Market Country Practice.

Bildeston Health Centre is a friendly, medium sized, rural dispensing practice. We are CQC rated GOOD and score very highly on the Patient Experience survey. We have a growing list size of 7,600+ patients with two GP partners. We currently have 2 practices nurses, 2 HCAs, and a very supportive care navigation and admin team.

We are active members of the South Rural PCN who provide us with a practice paramedic and clinical pharmacist.
