Burlington Primary Care is seeking an experienced, motivated, and compassionate lead practice nurse to join our friendly and forward-thinking practice team. Experience in primary care nursing and long term condition care is essential.
As lead practice nurse, you will play a pivotal role in shaping and developing nursing services within the practice. You will combine hands-on clinical work with leadership responsibilities, ensuring that our patients receive safe, effective, and person-centred care. Working closely with the GP partners, practice manager, and wider multidisciplinary team, you will contribute to service improvement, clinical governance, workforce development, and the achievement of practice and primary care network objectives.
The successful candidate will be an excellent communicator with strong organisational skills, a commitment to professional excellence, and the ability to inspire and support colleagues. You will lead by example, promoting a positive team culture and helping to drive innovation and continuous improvement across our clinical services.

Key Responsibilities
- Providing clinical leadership and day-to-day support to the nursing team.
- Delivering high-quality patient care within your scope of practice.
- Leading on long term condition management across the nursing and HCA team.
- Supporting workforce planning, recruitment, induction, training, and appraisal of nursing staff.
- Ensuring compliance with CQC standards, national guidance, and practice policies.
- Developing and implementing nursing protocols, procedures, and patient care pathways.
- Working collaboratively with GPs, allied health professionals, PCN colleagues, and external agencies to improve patient services.
